About LMC

Residential Supports:

LMC offers three levels of support for people living in a residential setting. Support staff are titled “ Resident Advisors”.

1). Twenty-four hour Supervised Home:

This level of support is the most comprehensive. Resident advisors are in the home twenty-four hours a day, seven days a week. These staff assist the people living in the home with their daily living skills such as bathing, cooking, taking their medications, cleaning, home décor, shopping, etc. Resident advisors schedule and attend medical appointments as well as plan social/community activities.

2). Semi-Independent Home:

This level of support is for people who need approximately 40 hours per week of service. The schedule for the resident advisor, and the level of support required for the person living in this home is determined by the needs of the person living in the home. Supports usually includes budgeting, shopping, attending some medical appointments, transportation to out of town events, etc.

3). Independent Living:

This service is generally for people who need less than 40 hours of support per month. These supports usually include help navigating Medicaid/Medicare, completing paperwork tied to benefits, attending certain doctor appointments.

Home Based Waiver Services:

LMC sends support staff into the home to help people who live with a family member or guardian. Support staff provide respite services for the family up to four times a month and habilitation services for additional hours per month. Support staff assist with daily living skills and social activities.

Focus Group:

This is a behavioral support team that helps develop supports for people who are experiencing challenging times and are in danger of losing their placement. Focus Group works with people who live at home, live in various residential settings, attend day programs or vocational programs.

Mental Health Supports: Supporting people with Mental Health diagnoses and helping them function in the community. Supports may include residential services, scheduling and attending doctor’s appointments, utilizing public transportation, and interacting with Medicaid/Medicare personnel.
